The zip file contains all forms necessary to begin the order process.

CD Digital Card will begin processing your order(s) when we receive an approved, Signed Credit application and all required forms.

Properly complete the following forms:

• Signed purchase order
• Data master disc (track at once and closed)
• Art (Film) Disc or electronic art work. If customer is providing film we require a
  matching print.
• Signed IRMA Intellectual Property Rights Form

Customer supplied printed packaging materials must be accompanied by:

• Packing list
• Print on each box - Job name - PO. Number, etc.
• Provide 5 to 10% more than the Disc ordered.
• Jewel case and DVD cover and insert must conform to CDDC’s template(s).
• Art, film and any packaging must arrive with master source material otherwise we will hold the job until all components are received. This will result in your order being delayed. Note: Art Resolution should be 300 DPI or higher. Also, you must provide Color call outs (PMS Color Numbers) ART: All "Fonts" should be converted to outlines (Curves) . Do not include files not to be used, no link files and make sure for OFFSET Printing that the color black is a combination of CMYK and not 100% black.
